SECTION 5: CONTINUOUS IMPROVEMENT PLANNING
PRIORITY 1 – ALL STUDENTS LEARN TO READ WELL
- By June 2025, literacy for Students with Disabilities (SWD) will improve, as measured by a 10% increase from their iReady September 2024 overall diagnostic reading score for 80% of 1st-5th grade 8:1:1 and 12:1:1 students with 75% attendance or above.
- By June 2025, literacy for Students with Disabilities (SWD) will improve, as measured by a 5-phase increase in Pyramid's Picture Exchange Communication System for 85% of Kindergarten 6:1:1 students.
- By June 2025, literacy for Students with Disabilities (SWD) will improve, as measured by a 5- point increase on the Communication Matrix for 80% of kindergarten through second grade 12:1:4 students.

PRIORITY 2 – ALL STUDENTS ARE PHYSICALLY AND EMOTIONALLY SAFE
By June 2025, physical and mental wellness for Students with Disabilities (SWD) will improve, as measured by a 1-level Increase on the P10X Behavior Continuum for 80% of 6/8/1211 kindergarten through fifth grade students.
By June 2025, physical and mental wellness for Students with Disabilities (SWD) will improve, as measured by a 10% decrease in the number of Behavior Support Paraprofessionals from the October 2024 baseline number of Behavior Support Paraprofessionals, per the SESIS Reports.
SECTION 5: CONTINUOUS IMPROVEMENT PLANNING
PRIORITY 3 – ALL STUDENTS HAVE A HIGH-QUALITY ACADEMIC EXPERIENCE
- P10X will address Priority 3: All students have a high-quality academic experience with the following SMART Goal: By June 2025, the 90% of 8:1:1 students in grades 3 – 5 will demonstrate a 4-chapter increase as measured by Math in Focus Chapter Tests.
- P10X will address Priority 3: All students have a high-quality academic experience with the following SMART Goal: By June 2025, 85% of 12:1:4 students in 1st-5th grade will master their Math IEP goals as measured by SESIS Progress Reports. 3. P10X will address Priority
- All students have a high-quality academic experience with the following SMART Goal: By June 2025, 90% of 6:1:1 students with disabilities in 1st-5th grade will demonstrate a 6-lesson increase in Connecting Math Concepts as measured by lesson performance and mastery tests.

PRIORITY 4 – ALL STUDENTS GRADUATE COLLEGE AND CAREER READY AND HAVE A STRONG PLAN AND PATHWAY TO ECONOMIC SECURITY
P10X will address Priority 4: All students graduate college and career ready and have a strong plan and pathway to economic security with the following SMART Goal: By June 2025, 50% of 6:1:1 kindergarten through fifth grade students will be toilet trained as measured by Assessment of Basic Language and Learning - Revised.

PRIORITY 5 – ALL DISTRICTS AND SCHOOLS ARE MORE INCLUSIVE AND RESPONSIVE FOR PARENTS AND FAMILIES, INCLUDING HAVING MORE FAMILIES CHOOSE NYC PUBLIC SCHOOLS
P10X will address Priority 5: All districts and schools are more inclusive and responsive for Parents of Students with Disabilities (SWD) with the following SMART Goal: By June 2025, 60% of P10X 5th Grade families will attend one “Safety, Transition, and Independence” meeting across the 24-25 School Year as measured by Parent Coordinator Attendance Reports.

QUALITY INDIVIDUALIZED EDUCATION PROGRAM (IEP)
By June 2025, to strengthen the quality and implementation of IEPs for Students with Disabilities, 80% of untenured teachers will write student’s Individualized Education Program (IEP) with 80% accuracy as measured by the P10X IEP Quality Review Checklist (adapted from the citywide IEP review document).
By June 2025, to strengthen the quality and implementation of IEPs for Students with Disabilities (SWD), we will improve quality and timeliness of IEP completion, 80% of untenured teachers will write student’s Individualized Education Program (IEP) with 80% accuracy, as measured by P10X IEP Quality Review Checklist.
